They are currently being paid their contract amount to let said HIV medicine rot in a warehouse where it will either spoil or get stolen by gangs

You would have to show that counterfactually, America causes more mass murder than if another country was in power. What little we saw of soviets and chinese maoists having a sphere of influence, we found that to not be the case. Then you can also look at what happens when other countries are in power over pro-Western ideologies and continue to see this not be true. And you can even see what happened in Europe when the totality of the European sphere of influence was greater.
So it’s not inconsistent at all. Supporting the West, particularly the Americans dominating over everyone else, is being against mass murder. Supporting the Soviets (as you do) is being pro-mass murder.
